PATIENT BILL OF RIGHTS & RESPONSIBILITIES

Westview Medical is committed to aiming for quality, preventative health care for patients and their families.

 

 

As a patient here at Westview Medical, you have the right:

  • To receive care that supports your physical, emotional, and educational needs with referrals to other resources as indicated

  • To appropriate, planned and coordinated care

  • To continuity of care

  • To be treated with dignity, respect and courtesy

  • To confidentiality (see privacy statement)

  • To receive answers to your questions and explanations about all aspects of care

  • To have access to information in the health record

  • To be informed of all expenses not covered by OHIP

  • To be informed of medical students and residents learning / providing care at the office

 

 

As a patient here at Westview Medical, you have the responsibility:

  • To be an equal partner in caring for your health

  • To book appointments in advance for regular check-ups and prescription refills instead of last minute requests

  • To book appointments for one person per appointment unless previously arranged, to devote enough time per person per issue

  • To communicate all known reasons for the visit when booking appointments to allow enough time for the visit or else book more visits

  • To communicate to staff if anticipating being late or if unable to attend that visit

  • To communicate directly with us regarding concerns and questions about our practice or our staff

  • To complete forms as much as possible and show them to our receptionists prior to seeing us

  • To ensure payment for all uninsured services required
