As a patient here at Westview Medical, you have the responsibility:

To be an equal partner in caring for your health

To book appointments in advance for regular check-ups and prescription refills instead of last minute requests

To book appointments for one person per appointment unless previously arranged, to devote enough time per person per issue

To communicate all known reasons for the visit when booking appointments to allow enough time for the visit or else book more visits

To communicate to staff if anticipating being late or if unable to attend that visit

To communicate directly with us regarding concerns and questions about our practice or our staff

To complete forms as much as possible and show them to our receptionists prior to seeing us

To ensure payment for all uninsured services required
