No-Show & Cancellations

No Shows

In order to provide efficient services for all of our patients, our office has a no-show and cancellation policy. The policy enables us to better utilize available appointments for more patients in need of medical care.

Patients may book appointments in person, over the phone/text, or via email. If patients are unable to attend an appointment, we ask that they contact our office promptly, preferably at least 24 hours in advance, to cancel or rebook their appointment. This allows enough time to fill the space for someone else in need of urgent care. Patients who cancel within the 24 hours may be subject to a no-show fee.

The no-show fee is $40.54. If the appointment was booked for 30 minutes or longer, the no-show fee is $74.53. Patients are expected to pay the no-show fee by their next appointment.

After a second no-show, patients will be required to pay BEFORE they can book another appointment. However, these patients are still welcome to attend walk-in clinics with Dr. Klassen without paying the fee.

Prescription Refills

Rx Refills

Westview Medical does not refill any medications without seeing the patient first. We do not accept prescription refill requests over phone/email, nor faxed in from the pharmacy. We try to give enough of the prescription to last until they next need review with an appointment. This policy ensures that patients are followed up on a regular basis and review whether this is the best treatment we can offer them.

In special circumstances, patients may be unable to come in to the office. Prescription refills without an appointment will be subject to a $20.00 prescription refill fee. This fee cannot be used consecutively, and typically will only cover a refill until the next available appointment time. Patients are expected to pay the prescription refill fee by their next appointment.

Forms for Physicians to fill out

Forms

There are a variety of forms that come to our office: for school, work, insurance, ODSP or OW, residency applications, travel grants, etc. You may drop them off in person or via email. To ensure that these forms get completed, patients are required to book an appointment. Likely they get completed within one visit, but longer forms (+4 pages) may require an additional visit.

Please note that some forms may or may not be covered by OHIP. Our common practice for uninsured forms is we bill the patients directly, and the third party (if applicable) can reimburse the patient. Please review our list of uninsured OHIP services or speak to reception if you have any questions whether or not there is a fee.

Health Card & Documentation

Health Card

Patients should bring a valid OHIP health card for every visit. In certain situations, valid health cards from other provinces may be accepted. If your health card is expired, our office may request that you visit the nearest ServiceOntario office to renew your health card, otherwise the cost of the appointment will be billed directly to the patient.

Other Uninsured OHIP Services

Anchor 1

The Ontario Health Insurance Plan (OHIP) provides coverage for a vast majority of physician services; however, there are some services that are not covered. Our physicians do offer a number of these services, but the responsibility for payment lies with the patient or the third party requesting the service.
